Job Responsibilities:
- Conduct individualized case management, including case conceptualization, goal setting, intervention planning, and monthly sessions with individuals and families to review progress.
- Collaborate with Probation Officers (POs), Child Protection Officers (CPOs), and other stakeholders such as families, schools, and employers for co-case management.
- Build partnerships with stakeholders, including existing and prospective employers, schools, CSO agencies, pro-social communities, and community partners to support reintegration efforts.
- Lead therapeutic group sessions for clients, including evening sessions from 7-9 pm.
- Prepare, attend, and present cases at internal and external case reviews to provide updates on intervention progress.
- Ensure thorough casework documentation and reporting in compliance with licensing requirements.
- Work with Hostel Operations & Development Department to provide essential information for scheduling daily movements and assist with other ad-hoc duties as needed.
Job Requirements:
- Diploma/Degree in Social Work with at least 2 years of experience of mentoring and working with youths
- Prior experience in working with youth-at-risk would be an advantage
