Job Description
- Assess physical and functional status and needs of clients with physical limitations resulting from stroke, traumatic brain injury, amputation and various neuromuscular dconditions
- Design and provide evidence-based and client-centred interventions (individual and groups) in collaboration with the multi-disciplinary team to improve client ability in functional domains, so as to promote reintegration to home, work and society
- Work collaboratively with a dynamic multi-disciplinary team to provide clinically effective service to achieve client health and vocational goals
- Educate clients’ families and employers about how to accommodate and support clients in their rehabilitation
- Ensure comprehensive documentation where all relevant information related to client care is recorded and available to all members of the multi-disciplinary team
- Perform field work involving visits to healthcare institutions, home and worksite visits to support clients’ transition into employment and the community
- Conduct public education and outreach for programmes and services
Job requirements/details
- At least 3 year(s) of working experience in the related field is required for this position
- Part-time (negotiable working schedule)
- Full-time ( 5 and half day work week, negotiable)
- High remuneration (Base + Incentives)
- Required skill(s): AHPC registered, Musculoskeletal and orthopaedic outpatient/private experience
- Confident in treating both chronic and acute conditions in a safe and appropriate manner
- Provide comprehensive documentation of client's condition and progress in timely manner
- Team player, willing to make contributions to the team in terms of cross-referrals to appropriate multi-disciplinary professions, creative ideas and social marketing
- Great communication and interpersonal skills, with the ability to work with patients from diverse cultural background and nationalities
