Job Description:

- Develop individualise care plans and programmes .
- Execute the care plan and monitor progress.
- Coordinate care for residents with relevant stakeholder.
- To complete all administrative tasks required for casework and counseling
- To be involved in setting up structures and processes within the department when necessary
- To lead ad-hoc tasks and projects by the agency when required
- To be able to work with partners and stakeholders when required
Job Requirements:
- Min Bachelors Degree in Social Work from a recognized institution
- Min 1-3 years’ work experience in social work, preferably in a social service or healthcare setting would be good
- Candidates with less experience will be considered for entry level
