Job Purpose

The Social Worker will be responsible for providing comprehensive support across various Singapore Red Cross programmes, including Community Befriending (ElderAid), Meals with Love, Young Hearts (YH), Medical Chaperone and Transportation (MCT) and Home Monitoring and Eldercare (HoME+). The incumbent will play a critical role in reducing vulnerability, enhancing resilience, and promoting the well-being of isolated seniors, lower-income families, and youth from disadvantaged backgrounds through direct service delivery, administrative support, and community engagement.


Job Responsibilities

Primary Responsibilities

  1. Assess referral cases on the suitability to SRC programmes
  2. Facilitate referrals of clients to various assistance schemes and community programmes.
  3. Provide advisory support for the programmes operation direction
  4. Establish and build professional relationships with client and next of kin (e.g. family, Care provider etc.)
  5. Identify, clarify and conduct needs assessment to assess the severity, issue(s), risks and protective factors for intervention.
  6. Adopt varying modes of engagement ( e.g telephone interview ; home visits) to engage and monitor the clients.
  7. Conduct assessments to identify the needs of programme beneficiaries and develop individualized support plans (all programmes).
  8. Develop and provide recommendations for consideration, monitor and evaluate the progress achieving our goal.
  9. Partner agencies to ensure smooth transition of cases

Stakeholder Management

  1. Engage with community stakeholders to promote programme visibility and support
  2. Develop and maintain relationships with external partners to facilitate referrals and support services (all programmes).

Administration

  1. Maintain accurate and up-to-date records of beneficiary interactions and programme activities (all programmes).
  2. Prepare regular reports on programme outcomes and impact for internal and external stakeholders (all programmes).
  3. Ensure compliance with organizational policies and relevant regulatory requirements (all programmes).

Any Other Duties

  1. Collaborate with other team members and volunteers to ensure effective service delivery (all programmes).
  2. Participate in professional development opportunities to enhance skills and knowledge relevant to the role (all programmes).

Job Specifications

Qualification and Experience

  1. Bachelor’s degree in Social Work or a related field.
  2. Registered Social Worker with the Singapore Association of Social Workers (SASW) is preferred.
  3. Minimum of 2 years of experience in social work or related field, preferably with experience in eldercare, family services, or youth development.

Additional Skills and Knowledge

  1. Strong interpersonal and communication skills, with the ability to engage effectively with diverse populations.
  2. Excellent organizational and time management skills, with the ability to handle multiple tasks and meet deadlines.
  3. Proficiency in Microsoft Office and experience with case management software.
  4. Knowledge of relevant community resources and social services in Singapore.
