- Take the lead in organizing the team to provide patients and their families with the right programs and services to guarantee the best possible health outcomes and improve at-home coping.
- Take the lead in organizing and directing community services, organizing follow-up visits (by phone or in-person), and educating caregivers in order to make sure that patients and their carers are able to manage after being discharged. Assisting patients and their families with end-of-life care planning is another aspect of it.
- Manage and oversee the systems for data entry, administration, and analysis.
- Monitor employee performance and guarantee the team's clinical competency.
- Arrange and supervise the team's training and professional development needs.
- To guarantee efficiency and high-quality care, review and amend work procedures in collaboration with internal stakeholders and outside organizations.
- Take part in or oversee hospital initiatives to raise the standard of clinical treatment, such as research projects, LEAN projects, or quality improvement projects.
Job Requirements
- At least a diploma/degree in social work, nursing, allied health, or any other clinical field pertinent to the position.
- At least 8 years of experience working in the healthcare industry
- Prior experience leading a team will be an added advantage
