Job Description:
- The Case Management Officer works with clinical departments to develop and implement clinical pathways and case management programmes and activities. These include assessing, planning, coordinating, monitoring and evaluating of clinical pathways and case management services.
Responsibilities and Duties
You will be responsible for the following:
- Screen all new admissions daily to identify cases for case management / receive referral from the ward.
- Assess and Identify patients' medical, psychological, social, environmental, vocational and financial needs.
- Synthesize assessment information to prioritize care needs and develop care plans.
- Formulate plans of patient care in collaboration with clinicians, other health care members, and patient/family.
- Implement, monitor and evaluate the patient’s response to treatment/care plan and revising it as needed.
- Integrate and coordinate the activities of multiple discipline services or resources needed by patients/families.
- Ensure patients' diagnostic tests, treatments, and consultations take place promptly and in proper sequence.
- Provide patient education, if required.
- Initiate, implement and coordinate the discharge planning process for patients timely and appropriately.
- Act as the point of contact and a liaison between the patient, family and the hospital.
- Identify system, practice, patient, and community issues and implement appropriate remedial actions to address deficiencies found.
- Provide education, information, resources and support related to case management functions.
- Establish a framework for data collection and analysis on care delivery process for variance management.
- Advocate for patients if required.
- Conduct training & education on case management for doctors, nurses and paramedical professionals.
- Provide support to QA and clinical services committees in functions such as organizing meetings, preparing relevant reports for meetings, recording minutes and ensuring that follow-up actions are taken after meetings.
- Conduct clinical audits for various clinical work processes, case notes audit / review, audits to look into problem cases / areas and ad-hoc audits as chosen by the various QA related committees.
- Conduct ad hoc audits/ surveys initiated by QA and clinical service committees.
Requirements
- Min Degree in Nursing
- Min 3 years of nursing experience
- Nurses who are keen in adminstrative duties are welcome to apply
