- Assist in carrying out administrative work in the delivery of advisory services to patients and their families.
- This includes processing of information and statistics on assistance through an electronic system.
- Executive care advisory may be required to gather information from patients and their families and assist them with the financial assessments procedures.
- Conduct assessment / home visits when necessary.
- Liaise with relevant agencies / authority / patients family.
- Assure that all referrals are attended to by relevant personnel.
- Liaise with nurses on patient-related matters.
- Assure that all referrals are attended to by self or relevant personnel.
- Supervise the operational work carried out by customer care coordinator when handling subsidy cases.
- Act as a channel of communication between management and the customer care coordinator.
- Handle difficult customer and communicate escalated issues to the Manager.
- Keep update records of patients ( electronic or written data)
- Up-to-date the relevant information of government’s policy with regards to the elderly care Services and funding.
- Maintain confidentiality of customer database.
Requirements:
- Minimum O Level education
- Minimum 3 years of relevant working experience
- Expertise in Social Work and/or Customer Service would be an advantage
