Duties & Responsibilities:
* Assess clients/family psychosocial and behavioural concerns and needs prior admission
* Work closely with AIC, MOH and other government agencies to provide appropriate community services to ensure continuity of care.
* Provides counselling services as applicable and assists in utilizing community resources
* Evaluates and assists with resident’s financial obligations, assessing appropriate resources and insurance coverage
* Facilitate ACP and PPC for residents and family members.
* Home visit to help resident/client to alleviate or resolve their financial-social problem.
* Update family member and visitors on the latest visiting guidelines in accordance to MOH advisory
* Submits documentation accurately and timely. Updates and maintains resident’s records and charts
* Assists with discharge planning from centre to homes and nursing homes
* Performs other duties as assigned
Requirements:
* Minimum Degree in Social Work or related discipline
* At least 2 years of relevant experience
* Social work in healthcare background preferred
* Good interpersonal and critical thinking skills
* Ability to work well in a team
* Self-starter and self-disciplined
* Preferably ACP and InterRai trained.
* Proficient in Microsoft Office, Teams, Zoom, etc.
