Job Title: PCN Nurse Counsellor

Responsibilities:
- Provide direct ancillary services, i.e., nurse counselling, DRP, DFS, spirometry, to chronic patients in PCN GP Clinics.
- Provide education, prepare, pre-empt and coach patients, caregivers and family members on disease processes, disease management, preventive health and health promotion to empower them in making informed decisions regarding patient self-management, maintain well-being and prevent complications.
- Work with individual patients to develop and customize individual health plans according to their needs.
- Encourage and empower patient towards self-management in achieving desired goals and management.
- Track, evaluate and analyse patient’s results.
- Plan and conduct telephone consultations with patients to monitor and assess needs.
- Assess patient’s needs and make appropriate recommendations or referrals, in collaboration with GPs, for patients requiring services, e.g., financial, social, etc, within PCN scope.
- Provide recommendations and collaborate with GPs on patient management.
- Share best practices and streamline workflows with all PCN stakeholders.
- Liaise with healthcare partners on patient care, e.g., GP clinics, polyclinics, hospitals, service providers.
Qualifications:
- Experienced Registered Nurses with SNB, with interest in community health.
- At least 2 - 5 years of working experience in a healthcare institution
- Nurses who possess a positive attitude, a caring heart, a passion for helping others, and the willingness to learn and grow.
Job Description:
- You will work as a community nurse in a network of clinics during office hours. No night duties will be scheduled.
- You will receive plenty of training opportunities. Training will be done in the form of didactic lectures, workshops, direct supervision, group supervision, self directed learning and on-the-job. You are required to complete all training provided by internal and external trainers. By the end of your core training, you are expected to be equipped with sub-specialty nursing skills and knowledge.
