Job Description:
- Collaborate with multi-disciplinary teams (MDT) to implement patient care based on assessed needs and health conditions.
- Facilitate patient admissions, transfers, and discharges, ensuring timely and appropriate discharge planning.
- Monitor and evaluate patient progress according to the care plan and readiness for discharge.
- Observe patients for changes in condition, recognize early signs of clinical deterioration, and escalate issues as needed for timely intervention.
- Actively participate in MDT rounds, promptly execute physicians’ treatment and investigation orders, and assist with bedside procedures and treatments.
- Communicate patient progress and responses to treatment with the MDT, demonstrating clinical skills, knowledge, and critical thinking.
- Assist the nursing team with problem-solving and escalate issues to Senior Nurse Manager (SNM) or Nurse Manager (NM) as necessary.
- Provide patient and family education on disease management, prevention, and rehabilitation, and assess understanding for effective self-care.
- Help department supervisors manage resources and maintain expenses within the approved budget.
- Review and monitor infection control practices, identifying areas for improvement and ensuring compliance.
- Plan and conduct in-services for nurses and students to enhance skills and maintain high-quality care.
- Support and lead new initiatives, collaborating with stakeholders to implement changes in nursing practices.
- Evaluate the clinical performance of new nurses and nursing students, providing constructive feedback and documentation.
Requirements:
- Min Degree or Diploma in Nursing
- Advanced / Specialist Diploma in Geriatric / Palliative / Medical Surgical is an added advantage
- Minimum 6 years of relevant hospital work / community experiences
- Experience as assistant nurse clinician
