1) Please outline the staff’s key areas of responsibilities
- Devise and review individualised care plans for clients.
- Supervise a team of care coordinators in the provision of care to clients.
- Oversee the daily operations of care coordinators and advise on measures to ensure clients’ holistic well-being.
- Oversee activities for clients to promote independence, health, wellness and quality of life and ensure compliance to relevant statutory requirements and organisational policies.
- Maintain up-to-date documentation of clients’ conditions and care treatments.
- Work closely with the centre manager to co-develop activities and events to promote the wellbeing of the clients.
- Meet the grants KPIs set forth by grant makers and assign individual KPIs to the team members.
- Lead and manage the care team in Brahm Centre @ MacPherson.
- Support Centre Manager in management of care team to drive AAC grant programs and outreach KPIs
- Develop care strategies that can also be implemented by other care teams.
- Train the team in clinical care knowledge to enhance their value add to the clients.
- Collaborate with other leaders to leverage on each other’s strengths and capabilities.
- Ensure team maintains proper documentation and secure storage of case files, referrals, and other related documents across various systems i.e. Salesforce, Acutenet, Integrated Referral Management System (IRMS)
- Ensure that attendance for all training is promptly entered into Salesforce
- Ensure that all clients' personal data is protected and used in accordance to Brahm Centre's Data Protection Policy
- To ensure that all required administrative reports and follow ups are completed within deadlines given by your line manager
- Any other administrative roles that may be assigned to you.
