Job Description:
• Providing assisted enrolment to patients
- Guiding patients to enrol through HealthHub app for patients with Singpass
•Onboarding patients to HealthierSG, including booking of Health Plan related appointments (doctor consultation and care coordinator appointments).
•Tagging patients on Queue Management System (QMS) for HealthierSG related visits
•Answering patients’ queries with regards to HealthierSG
•Collecting of HSG Enrolment data
•Monitoring the stock level and topping up HSG related collaterals
- Guiding patients to download the app
- Guide patients in navigating the function available on the app such as
i.Appointment management
iiVideo Consult
iiiManage care plan and access health record
ivBook activities & programmes (HSG – social prescripting)
vPay bill and many more
