The role
The Staff Nurses collaborates closely with the doctor and social worker to provide home-based nursing service to the frail and home bound elders to meet the Foundation’s mission and operational targets, in accordance with the standards of care and service protocols established by the Foundation and the community.
Key Responsibilities
- Conduct comprehensive nursing assessment regularly to senior clients.
- Able to triage client condition and respond in a timely manner to address the issue.
- Be part of the interdisciplinary team to develop and implement client’s care plan.
- Work with the doctor to empower the patient and/ or caregiver in health improvement and disease management.
- Evaluate outcome interventions and revise care plans as needed
- Manage and monitor client’s medical and nursing needs as required.
- Maintain documentation of client’s progress and outcomes of care provision.
- Assist Doctor in charge and perform nursing procedures as needed.
- Assess and provide appropriate education and training for clients, caregivers and team members.
- Carry out necessary administrative duties in line with internal and external agency guidelines and SOPs.
Key Requirements
- Professional Certificate/Specialist Diploma/Advanced Diploma in Nursing, preferably with training in gerontological nursing.
- State registered with the Singapore Nursing Board (SNB) and possess a valid Practising Certificate in Singapore.
- Preferably 2-3 years working experience in the social services or community care sector.
- Extensive clinical knowledge and skills.
- Flexible, energetic and able to multi-task.
- IT proficient
- Independent team player with a penchant for working with the elderly and caregivers.
- Bilingual in 2 or more local languages as this role is required to interact with local elder person in the work environment.
Salary is indicative and commensurates with qualification and experience.
