The Renal Coordinator is responsible in educating mild chronic renal failure patients and end stage renal failure patients in self-care and teaching patients at risk of developing renal failure on retardation or prevention of the disease. The role involves:
- Providing patient education for chronic kidney disease patients
- Presenting treatment options for moderate to end stage renal failure patients
- Providing financial counselling for new dialysis patients or patients who are going to initiate dialysis
- Providing counselling service
- Patient case management
- Coordinating of dialysis plan
- Monitoring of Ambulatory Blood Pressure
- Maintaining and updating of CKD (Chronic Kidney Disease) and ESRD (End Stage Renal Disease) Registry
Requirement
- Bachelor of Health Science or Nursing
