Job Summary
Purpose of job
Provide leadership and oversee service delivery, training for volunteers according to programme objective in accordance to ethical and legal guidelines.
Key Responsibilities Areas
- Work closely with the Team Leads to identify potential clients who are in recovery according to programme requirements.
- Work with Crest Team Lead to develop strategy to operationalise plans in volunteer recruitment, design of training materials, befriender training, budget planning, execution and evaluation of programme.
- Provide care coordination: Develop and monitor care plans, referral and follow-up for clients and caregivers who are at-risk of mental health and dementia.
- Plan and conduct outreach and screening to the community, including community mapping, to promote mental health awareness and early identification of mental illnesses and dementia.
- Plan and organise cognitive stimulation activities or programmes for the community to support clients who are at-risk of cognitive decline, mental health conditions and dementia.
- Coordinate and match potential volunteers to clients to maintain social connectedness through befriending the clients.
- Plan and conduct training for volunteers in their roles and responsibilities to carry out befriending, and identifying red flags and mental health symptoms.
- Networking with relevant stakeholders to promote the interests and services of Filos Community Services.
- Documentation of visits and sessions conducted by volunteers to clients to meet the programme KPI.
- Provide monthly administrative reporting according to service requirement, and manage the data base of clients and volunteers.
- To undertake duties assigned by the Executive Director, Head and Team Leads.
Requirements
Experience:
- Knowledge in programme implementation, volunteer management and training.
- Experience in case management, groupwork and community work will be an advantage.
- Experience and/or knowledge in working with clients on Mental Health challenges and Person-Centred Care and recovery will be an advantage.
- Good organisational skills, highly motivated and service mindset to serve the community.
- Ability to work independently and as a strong team player.
- Good administrative and computer skills with proficiency in MS Office.
- Self-motivated, pleasant and resourceful personality.
- Ability to work well with a diverse group of clients, volunteers and stakeholders.
- Ability to carry out presentations and training to volunteers and stakeholders.
Qualifications:
Grad Diploma/ Degree in Social Work
Others:
Required travelling to different locations within service boundary.
