KEY RESPONSIBILITIES
1. Interview and counsel residents and their families to help them alleviate or resolve their medico-social and financial issues by:
1.1 Screening the social background of all new applicants and identifying potential cases that need assistance (financial and/or social).
1.2 Liaising with government and voluntary agencies, churches (where appropriate) when there are areas of needs.
1.3 Seeking financial support for residents who are unable to pay for nursing home stay.
1.4 Process Singapore Nursing Board, work pass applications and with any other professional or government bodies.
1.5 Prepare onboarding matters, maintaining of p-files and HRIS system
1.6 Conduct HR orientation
2. Ensure healthy bed occupancy and conduct basic assessment of suitability of patients/candidates to be admitted into nursing home
3. Joint consultation with medical staff, nurses, other healthcare professionals to discuss plans for total patient management. Participate in patient care conference and family conference
4. Liaise with organisations that provide services (e.g. public assistance, home rehabilitation care services) to meet the needs of the residents and their families
5. Apply for Medifund Assistance and/or other financial assistance for cases that need such help
6. Maintain computerized case records and statistics on residents. The records kept include individuals who have been rejected by family or burnt-out cases
7. Field work involving visiting residents at wards, job placement work places, institutions, other social agencies to help patients alleviate or resolve their financial-social problems
8. Prepare social and other related reports for management and other related institutions agencies on request by the ED/Head of MSW
9. Undertake to be well informed on government guidelines on healthcare and social services available within community as well as the latest in the field of social work
10. Initiate new support services for ex- residents and/or caregivers
ADDITIONAL DUTIES
11. Assist with work distribution within the department
12. Supervise, coach and appraise the MSW department staff
13. Participate in the training/orientation new staff and volunteers
14. Participate in internal and/or external committee/workgroup activities as well as research work as directed by the Senior MSW or the ED
15. Cover the duties of the other staff (MSW, MSWA) in his/her absence
JOB REQUIREMENTS
· Bachelor’s Degree in Social Work
· At least 5 years of experience in Healthcare / ILTC social work settings, with supervisory role
· Experience in setting up nursing home and loading of residents
· Ability to handle sensitive and confidential information with discretion
