Social Worker
Job Description
The Social Worker will provide support and interventions to persons with disabilities at the Enabling
Business Hub. He/She is committed and service-oriented and works in collaboration with other
agencies and partners to support persons with disabilities in providing training and job placement
support. He/She will work closely with other Allied Health Professionals in supporting persons with
disabilities towards gainful employment.
Job Responsibilities
• Perform stakeholder engagement and partnerships
• Conduct needs and intake assessments
• Perform case planning and goal setting
• Implement case plans and interventions
• Review casework interventions for improvements
• Conduct closure of cases upon achieving sustainable service outcomes for clients
• Maintain case notes and documents and prepare social reports
• Plan, facilitate and evaluate group work interventions
• Conduct environmental scans and employment-related need assessments
• Support, implement and evaluate employment-related development initiatives
• Plan, implement and evaluate programmes to address service gaps
Candidate Requirements
• Technical Competencies:
o Ability to conceptualize and formulate relevant and realistic case plan;
o Ability to engage stakeholders to develop solutions in community work;
• Core Competencies:
o Able to work well with team members to deliver employment outcomes for clients;
