- Assess seniors' care needs and formulate individualized care plans.
- Develop and implement programs for the elderly through regular data analysis, coordinate care with stakeholders, and document case notes.
- Facilitate networks, service referrals, participate in case discussions, and provide case management.
- Implement and evaluate new programs, review and initiate improvements.
- Guide volunteers, manage budgets and resources, and collaborate with other organizations to deliver programs effectively.
Job Requirements :
- Diploma/Bachelor’s Degree in Social Work.
- Preferably relevant experience in Social Service sector, working with elderly clients or any related field.
- Proficient in Microsoft Office (MS Word, Excel and PowerPoint).
- Able to work independently and in a multidisciplinary team to meet deadlines.
- Good interpersonal and communication (oral and written skills).
- Good knowledge about the community resources to engage stakeholders.
- Possess some knowledge of programme design, implementation and evaluation.
- Willing to work Mon to Fri 9am - 6pm.
