Our client is among the largest senior day care, nursing home and home personal care provider in Singapore. They are looking for Staff Nurse for their Home Care Team.
Job Scopes:
- Assess and record comprehensive patient history.
- Perform physical examinations.
- Develop and implement care plans.
- Conduct follow-up reviews, coordinate, and evaluate care.
- Provide specialized education and caregiver training.
- Implement and coordinate individualized and group learning programs.
- Evaluate the effectiveness of patient education.
- Conduct health education.
- Communicate treatment plans with patients and families.
- Assist in monitoring and maintaining infection control practices
- Educate patients and relatives on infection control
- Offer clinical guidance to Junior Enrolled Nurses, Nursing Aides, and Care Associates.
- Engage in ongoing professional development through training, supervision, and research.
- Perform additional tasks as assigned by supervisor
Requirements:
- Diploma / Degree in Nursing
- Advance Diploma in Geriatrics, Medical-Surgical, Chronic Disease Management, Community Health preferred
- Holds valid practicing certificate with Singapore Nursing Board
- Valid BCLS & AED certification/recertification
- At least 2 years' experience nursing experience in Singapore
- Knowledge in community care
- Willing to travel around Singapore
