Roles and Responsibilities
- Provide counseling service to patients and care givers
- Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
- Assist patient to understand the diagnosis and implication on ADL functioning
- Walk through with patients and care givers and provide emotional support and motivation for them
- Manage patients expectation and set realistic and achievable goals
- Arrange for interim care or make referrals for long term step-down facilities when necessary
- Participate in ward rounds and case conferences to represent patients' social, psychological and economic circumstances.
- Responsible for Volunteer Management
- Plan and oversee the volunteer programs, not limited to recruitment, screening, coordination, training and retention of volunteers
- Undertake projects and any other duties as assigned by the Management
Requirements
- Master/Bachelor/Post Graduate Diploma in Social Work
- Registered Social Workers with Singapore Association of Social Work (SASW) - will be an advantage
- Compassion and care for patients
- Good team player with positive attitude, caring and compassionate individual
- Good interpersonal skills, able to communicate people at all levels such as patient’s families, government agencies and other professionals
