A. Information & Referral
i. Conduct intake assessments.
ii. Provide appropriate information and refer clients to relevant agencies for further intervention.
B. Casework and Counselling
i. Manage assigned cases and case referrals.
ii. Conduct assessment, planning and intervention for cases.
iii. Develop and review appropriate intervention strategies.
iv. Participate in and/or conduct case conferences.
v. Document and evaluate case outcomes.
vi. Complete case recordings and update client information and documentation in the case management system.
vii. Provide supervision, guidance and advice to support staff, volunteers and student interns.
C. Group Work and Community Work
a. Programme Planning & Implementation
i. Plan, develop and implement relevant programmes to respond to the needs of clients.
ii. Advocate for appropriate resource allocation in the community.
iii. Network and collaborate with other community stakeholders.
b. Programme Evaluation
i. Evaluate and document programme outcomes.
ii. Identify trends, gaps or changes needed in provision of Centre’s services in the community.
D. Undertake any other responsibilities and duties assigned.
