Job Description:
Provide safe and accurate medication dispensing and comprehensive medication education/counselling to patients and caregivers
Perform medication reviews and interventions to prevent and/or resolve drug-related problems with appropriate therapeutic recommendations to enable safe and cost-effective medication therapy
Perform clinical assessment of patients/customers with minor ailments and provide Pharmacy-only medications (Pmed) and/or General Sales List (GSL) with appropriate education and advice
Handles research drugs and sample drugs in outpatient pharmacy
Communicate effectively within the inter-professional teams and the department, as well as outside of team/department (e.g. patients, caregivers, community partners, admin support staff)
Adopt patient-centred care approach to patient care and establish collaborative therapeutic relationships with the patients/caregivers
Exercise situational awareness in performing routine patient care activities in outpatient pharmacy, and communicate with colleague to uphold patient safety and clinical care quality standards in patient encounters
Manage the continuous improvement of clinical quality, medication safety, and operational efficiency in outpatient pharmacy
Assist in the conceptualisation and implementation of departmental, institutional, and cluster strategies in alignment to national policie and best practices
Assist in the management of outpatient pharmacy operations (e.g. prescription processing, patient education, inventory management) and the corresponding performance, quality, and value-based indicators
Supervise the outpatient pharmacy team and manages clinical rosters and allocation/scheduling of duties in collaboration with relevant stakeholders (e.g. pharmacy technician executive)
Supervise the medication delivery services and manages its team of staff
Provide supervision over junior pharmacists and pharmacy technicians/assistants in routine patient care activities in outpatient pharmacy
Comply to the prevailing pharmacy practice standards and institutional policies, and escalate to supervisors for advice appropriately
Investigate and manage medication-related incidents in outpatient pharmacy
Exercise appropriate change management principles in the areas of work
Assist in the management of patient safety, quality improvement, and clinical quality standards in pharmacy department and the
corresponding activities (e.g. ERM/CRM, PSQI indicators/audits)
Participate in the clinical education of students, trainees, and healthcare professionals
Participate in the staff training of pharmacists and pharmacy technicians/assistants
Lead and participate in medication safety and quality improvement activities in collaboration with relevant
Any other duties as assigned
Job Requirements: Min Degree in Pharmacy & VALID AHPC & min 5 yrs of relevant experience
