Job Description:
As a Primary Care Coordinator at I-CARE PCN, you will play a vital role in enhancing patient care and ensuring efficient service coordination. This position is ideal for those who excel in healthcare management, enjoy supporting medical staff, and are committed to promoting health initiatives.
Responsibilities:
- Coordinate patient care across various services to ensure seamless service delivery.
- Provide administrative and technical support to the clinic team.
- Educate, assist, and empower patients and caregivers by providing comprehensive health management guidance and lifestyle advice.
- Analyze patient data to identify trends and improve healthcare services.
- Collaborate with medical professionals to optimize patient outcomes.
- Participate in and support health promotion initiatives, such as the Healthier Singapore (HSG) program.
Qualifications & Skills:
- Diploma or Degree in a relevant field; fresh graduates are welcome.
- Experience in healthcare or social services, with additional retail or EPOS (Electronic Point of Sale) systems experience considered a plus.
- Proficiency in data analysis and MS Office.
- Excellent communication skills.
- Fluent in English, Chinese, and a local dialect.
- Proactive team player with a positive attitude and strong multitasking abilities.
Working Hours:
Monday to Friday, with an alternate half-day on Saturday.
