Job Overview:
The job-holder is responsible to ensure high standards of community care to vulnerable pregnant mothers and their newborn babies. Medical Social Workers perform a very important role by conducting home visits in order to perform intake assessments of the family’s social needs, identify risks to the mother and her newborn in order to provide the best community support for these underprivileged families.
They are expected to lead and supervise a team of Case Management Officers in a multi-disciplinary outpatient and community setting. He/She is also expected to work closely with and to provide and receive clinical inputs from the obstetricians, paediatricians, psychiatrists, dietitians, nurses and other members in the organisation.
Duties and Responsibilities
- Intake and Assessment: To conduct FAST (Family and Adult Tool Assessment) and ACE, (Adverse Childhood Experiences) via a home visit upon enrolment of a pregnant mother. Assessment of home environment risks through home visits.
- Counselling: To provide counselling and emotional support to patients and family members where needed to help with their coping
- Casework: To work with the team on case management of cases via huddles and multidisciplinary case conference. Case direction and assessment of risks for cases. Facilitation of weekly huddles to ensure accountability and have an overview of all cases. Facilitation and advocacy of needs through the monthly multi-disciplinary meetings to ensure holistic management and communication of higher risk cases. Interagency case conference to communicate and advocate for patients' needs.
- Direct support for crisis cases or high-risk cases by providing priority response to families in personal crisis including attempted suicide or have vulnerable children victims of abuse
