JOB DESCRIPTION
- Full spectrum of case management from intake assessment interviews to follow-up with discharged clients.
- Conduct case management and behavioral care for person with dementia and their carers.
- Provide casework support that includes but not limited to duties like information and referrals, liaising with external agencies, case recordings.
- Liaise with other colleagues within social services and with professionals in hospitals/agencies/organization to commission appropriate services to facilitate agreed plans for persons with dementia and their carers.
- Keep appropriate written records and produce reports, as required, in accordance with departmental policies and procedures: Database administration, Data entry for statistical compilation and filing.
- Prepare reports, attend and participate in Case Conferences, Planning and Review meetings as required.
- Conduct and facilitate group sessions for persons with dementia and the carers.
- Organise and facilitate activities for carers and or persons with dementia.
- Perform necessary administrative tasks required for funding purposes.
- Other duties as assigned.
REQUIREMENTS
- Degree in Social Work and registered as an Accredited Social Worker.
- Keenness in doing case management work with persons with dementia and caregivers.
- Team player and able to work as a group.
- Attention to details and able to deal with sensitive and confidential information.
- Experience working in dementia care, geriatrics or mental health settings preferred.
