Job Description:
- Establish Individual Care Plan (ICP) of clients and implement intervention plans
- Assist in identifying clients with early onset of medical conditions
- Conduct group/individual activities and provide care for the clients
- Guide/assist clients in the activities such as exercises, games, handicrafts, outings
- Assist with the transportation and escorting the clients to, from and within the centre
- Serve meals and provide personal hygiene care
- Measure and monitor clients' vital signs such as blood pressure , heart/pulse/respiratory rate
Requirements:
- Certificate/ training in Healthcare preferred
- Relevant working experience, preferably in social service sector or healthcare industry an advantage
