Care managers are registered nurses specially trained in chronic disease management in primary care setting. The care manager works closely with your doctor, advanced practice nurse, allied health members and yourself to manage your chronic conditions.
Job Description
- Perform relevant clinical assessment and evaluate the patient’s current disease control.
- Assess patient’s concerns about his/her condition and learning needs.
- Explore for possible precipitating factors for sub-optimal/poor control.
- Reinforce patient/caregiver’s education and self-management skills.
- Develop/evaluate the plan of care to improve disease control with patient/caregiver.
Requirements
- Min Dip in Nursing
- Possess adv dip or specialist dip in EITHER chronic disease / mental health / endocrine / diabetes / gerontology
