Job Description:
- Coordinating and providing care activities for a group of chronic disease patients.
- Perform basic preventive health screening and health assessments
- Provide health education to patients on home monitoring devices such as home blood pressure, glucose and weight monitoring
- Document all care activities rendered to patients accurately.
- Utilise IT systems to track patients’ appointments, laboratory tests and workshops, vaccination needs
- Planning, organizing and coordinating workplace health promotion activities for staff and perform any other duties as assigned by supervisors.
Job Requirement:
- Minimum GCE 'O' Level
- Basic knowledge in Microsoft Office applications - Excel, Word, PowerPoint
