Locum Pharmacist (Min 3 Days per week, Up to $55/hr) *OUTPATIENT* @WEST #HAL

Salary: up to $55/hr

Working Days & Hours:

5.5 days Office Hours (any 3-4 days a week)

Location: WEST

Job Description:

1. Review each patient’s medication chart or prescription for the appropriateness of the drugs prescribed and to contact the prescribing doctor when an intervention is needed. The review has to ensure that:All orders are clear and specific
   Doses are prescribed within therapeutic limits
   Any potential adverse reactions and interactions are identified
2. Fulfil all drug orders accurately and in a timely manner.
3. Ensure that all related costs and revenue are accounted for, and accurately recovered.
4. Dispense medications to patients, explaining the indications for drug therapy and instructions for administration.

Job Requirements:

• Min Bachelor Degree in Pharmacy
• Registered with SPC
