This position reports to the Deputy Director of Clinical Transformation and will support the War on Diabetes, Community Intervention Programmes (WOD-CIP) in project/ programme coordination, implementation, operationalisation and communications with external and internal stakeholders. This includes but not limited to Tiered Weight Management (TWM), its workgroups and related events/ projects/ programmes as assigned. Hence, he/ she is expected to be a good team player, working closely with various institutional / zonal clinicians and administrators, etc.
Job Description:
· Support the program team to ensure that program activities will be implemented on time, scope and within budget, achieving the related performance objectives.
· Undertake and prioritize the day-to-day tasks for the smooth operations of the program, manages all internal delivery, community partners, third party providers (contractors) service delivery, where necessary. This will include, coordinating the logistics support, facilities and supporting the administrative aspects of the various intervention workshops.
· Facilitate and coordinate the delivery of the program in the community and provide logistics and marketing support to encourage outreach, uptakes leading up to a successful roll out of the various workshops and supporting events.
· Handle ad-hoc assignments when required including helping out in programmes and events as assigned.
· Ensure that key data components are captured and collected in a secured and organised manner throughout the program at the appropriate points so that the necessary reporting and subsequent evaluation of the program can be done.
· Be a point of contact for our targeted population to coordinate outreach, participant recruitment and feedback and ensure that follow up is carried out in a satisfactory manner.
· Fosters a strong working relationship between all stakeholders in the program, including partners/providers, the clinical staff, administration and information management staff.
· Ensure compliance to standards and processes to preserve patient privacy, confidentiality, and the security of health information.
Job Requirements:
· Minimum degree/diploma holder in any major
· Experience in the healthcare industry will be an advantage
· Preferably possess written, analytical, communication and presentation skills
· Proficient in various computer applications, including Microsoft Word, Excel & PowerPoint
