The Case Manager provides leadership and oversees the COMIT team to ensure service delivery is in accordance to the intended programme objectives and care model for COMIT programme as outlined under the terms & conditions of the Letter of Award/Addendum.
They maintain an overview of the service planning and implementation of the COMIT programme. The Case Manager will work closely and provide guidance to the COMIT team to achieve the COMIT goals of timely assessment, provision of intervention and support eg. psychoeducation, case management and counselling, as well as caregiver support and training.
In addition, the Case Manager will also establish and/or maintain networking with other community partners or agencies, for purpose of collaboration and to facilitate co-management of cases.
This candidate will be one of the key members driving the COMIT project.
Programme Development and Implementation
- Plan, implement and monitor the outreach, care process, referral and follow-up workflow.
- Provide case supervision to the team to provide intervention and support, in alignment with the overall COMIT programme objectives, adhering to the relevant ethical and legal guidelines.
- Responsible for establishing the network with other stakeholders like community partners and/or related agencies to build collaborative partnerships and better facilitate care and support for clients and caregivers.
- Establish and review work and communications workflow including identification, referral and escalation work processes as well as the development/update of the COMIT service manual.
Administration and Staff Development
- In charge of the analysis and review of the collated data, and monitoring of the team’s progress and performance.
- Ensure programme data and documents are kept and maintained, in accordance to the COMIT programme requirement, as well as the prevailing personal data protection and security guidelines.
- Oversee the training and professional development of the COMIT staff team with relevant competencies to provide the appropriate care and support for clients and caregivers, aligning to the intended care model of COMIT. This includes ensuring all new/existing staffs are clear on their job scope and responsibilities.
- Ensure that the team practices are in adherence to the prevailing organisational, ethical, safety and legal requirements, where relevant.
Other Duties
- Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.
- Open to a 6-month contractual role
- Recognized Masters degree/ Bachelor's degree in Psychology, Counselling or Social work.
- Has at least 5 years of working experience with case management, preferably in the social service sector.
- Team player with good interpersonal, oral and written communication skills.
- Preferably with supervisory experience, able to provide case supervision to the team to provide intervention and support through the project.
- Work experience in the COMIT/CREST or social service sectors would be an advantage.
