The Job
This role is responsible for overseeing Programme Services and Afterlife Care of our elderly beneficairies.
RESPONSIBILITES
- Possess a thorough understanding of Charity’s programs and services.
- When necessary, liaise with relevant bodies to arrange for the member's last rites.
- Create, maintain, and update accurate case files and database of each Afterlife Care members
- Conduct home assessments for potential Afterlife Care members
- Handling of Afterlife Care walk-in or telephone enquiries.
- Following up with cases reported from the hotline or referral from government agencies.
- Explore avenues for the expansion of the Afterlife Care external parties.
- Implement community outreach programs and new projects of eldercare
- Organise and facilitate an elderly befriending program.
- Conduct periodic case reviews and ensure proper documentation
- Refer and coordinate care services, ensuring adequate support to Afterlife Care members.
- Adopt a case management approach when working with beneficiaries, i.e., network with other agencies/specialists to provide multidisciplinary assessments and monitor beneficiaries.
- Prepare monthly reports on the progress of the Afterlife Care programme(s).
- Ad-hoc assistance in the Charity’s events
Requirements
- Minimum Diploma in Social Work or related disciplines
- Prior experience in Social Service setting is preferred
- Able to engage, assess, and provide intervention independently for care plans
- Knowledge of eldercare services, regulations, and practices.
- Ability to work independently.
- Compassion, empathy, and a genuine interest in elder care and charity work
- Proficient in MS Office Applications
- Able to work on some evenings and weekends as needed
- Bilingual in English and Mandarin. Ability to speak dialects will be an advantage
