We are looking for a motivated Care Coordinator to improve patients’ quality of life. Duties of the Care Coordinator include working with multiple patients in managing their therapy and appointments. The ideal candidate should be highly organised and efficient. If you have previous experience in hospitals and clinics, we would like to meet you.
Responsibilities:
- Welcome patients at our centre before offering assistance.
- Systematize a care plan for several patients at once, while coordinating with diverse healthcare providers who hold varied schedules.
- Prepare and provide educational materials to help new patients understand their medical conditions and the various possible outcomes of their care.
- Make phone calls or send e-communications to patients, healthcare professionals, health coaches and other medical specialists to schedule consultations or medical procedures.
- Work out barriers that could prevent patients from getting adequate healthcare.
Requirements:
- Proficient in MS Office (Excel and PowerPoint, in particular)
- 3 years of administrative assistant experience in a healthcare environment
- Familiarity with email scheduling tools
