We are seeking dedicated and compassionate Senior/Staff Nurses to join our Home Care team. Reporting to the Nurse Manager, you will deliver patient-centered care that meets the unique needs of home care patients while adhering to the highest standards of nursing practice, statutory requirements, and organizational policies. You will also play a key role in mentoring and supervising junior staff, contributing to the development of a skilled and cohesive nursing team.
Key Responsibilities:
- Provide safe, competent, and individualized nursing care, ensuring proper documentation, treatment, patient monitoring, and infection control practices.
- Manage complex patient care, utilizing clinical skills and critical thinking to support advanced disease management.
- Monitor, reassess, and adjust care plans based on patients' evolving conditions, offering support to both patients and their families.
- Serve as a liaison between physicians, patients, and families, coordinating care and offering emotional support.
- Assess the home care environment, recommending and allocating necessary equipment and aids to enhance patient comfort and living conditions.
- Administer medication, perform wound care, and ensure continuity of care through thorough follow-up and documentation.
- Actively participate in Multidisciplinary Team meetings, collaborating with various teams to ensure seamless care delivery.
- Maintain strict confidentiality regarding patient care and manage medical supplies and equipment efficiently.
- Stay informed on industry guidelines, maintaining professional standards in line with the Singapore Nursing Board's Code of Ethics and Professional Conduct.
Requirements:
- Diploma in Nursing from a recognized institution and registered with the Singapore Nursing Board.
- Post-registration experience in hospital or hospice care preferred.
- Proficiency in IT systems and documentation.
- Passionate about providing high-quality patient care and customer service.
- Strong communication, interpersonal, and time management skills.
- Ability to work well under pressure and as part of a team.
- Preferred candidates will have specialized experience in Oncology, Palliative Care, Gerontology, or Geriatrics. Advanced diploma or equivalent experience in these fields is a plus.
