Location: Jurong and Chai Chee
Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.
Job Responsibilities
- Provide financial counseling during patient admissions.
- Coordinate assessments to evaluate nursing, rehab, and social needs.
- Review patients' participation in activities, visitor interactions, and financial status.
- Offer therapy, nursing, and social recommendations to support home discharge or other care plans.
- Assist in securing financial aid for home care equipment, as needed.
- Counsel patients with psychosocial issues and facilitate family discussions on financial concerns.
- Collaborate with the Nursing team to assess residents' care and social needs.
- Assist residents and families with financial aid applications (e.g., Means Test, Medifund, IDAPE).
- Update resident profiles, social reports, and financial status.
- Supervise and guide the case management team.
Job Requirements
- Degree in Social Work or related discipline
- At least 5 years of relevant experience
- Social work in healthcare background preferred
- Good interpersonal and problem-solving skills
- Caring, positive and self-disciplined individual
- Ability to work well in a team
- Proficient in Google Workspace (i.e. Gmail, Google Docs, Sheets, Slides etc)
Salary and Renumeration
- $4500-$5000 (base on experience, can be higher if very experienced)
- 1 month AWS + 2 months Performance Bonus
