Case Manager

Core Function:

The Case Manager is responsible for providing casework services for clients of SUN-DAC and their families.

Core Responsibilities:

1. Ensures a smooth process for clients’ enrolment to SUN-DAC and assist in the means testing application process via the HOMES system.

2. Conducts case-conferences with SPED schools and other social service agencies.

3. Uses an evidence-based approach for effective delivery and evaluation of casework services for clients.

4. Monitors, evaluates casework assessment and ensures all assessments are able to meet stated objectives and outcomes.

5. Attends to client’s issues and work with Allied Professionals to plan for intervention.

6. Submits timely assessment and evaluation reports of clients.

7. Involves and assist in the yearly audits.

8. Submits timely monthly reports and claims to SGenable.

9. Conducts home visits to assess clients or provide emotional support for caregivers.

10. Organises and/or conducts caregiver meetings and caregiver appreciation events.

11. Develops long term care plans and provides counselling for caregivers.
