The Challenges
You will work within a multidisciplinary team to ensure continuity of holistic patient care, by assessing and evaluating needs of the patient, and planning, coordinating, implementing and monitoring the solutions to address those needs and to participate in other clinical care activities.
As a Case Manager, you will be expected to:
- provide case management service to patients who are admitted to the ward and discharged
- track patients who have exhibited past high-risk behaviours or at risk of becoming revolving door patients and/or other special needs
- track and provide follow-up case management in the outpatient setting
- provide supportive therapy, counseling and psychoeducation
- home visits
The Requirements
- Degree in Psychology preferably, but other degrees will be considered.
- Working experience with individuals suffering from psychiatry disorder will be of added advantage
- Must be a team player with passion in patients’ care, willingness to learn, and able to contribute to the team.
