Care@Community Department aims to deliver holistic care that is centred on an individual’s needs. It mainly targets seniors that are frail, whom have multiple care needs, that will become increasing complex with time. With the integration of community-based aged and social care services, it enables the seniors to continue to stay in the community for as long as possible, therefore allowing them to age-in-place.
Responsibilities:
· Provide Advance Care Planning Facilitation to the public.
· Conduct assessment of client’s needs.
· Provide Bereavement Counselling and/or therapy to clients and families; prepares treatment plans, discharge plans, and follow-up care programs; provides crisis intervention and emergency services as required
· Monitor and evaluate the effectiveness of counselling interventions and adjust strategies as needed.
· Conduct Peace/Cloud Café sessions
· Develop and review appropriate protocols, polices and procedures, in care process, referral and assessment.
· Expand existing service counselling provision by promoting organization’s counselling and training services among other agencies and potential service users.
Requirements:
· At least a Bachelor’s Degree in Counselling.
· At least 3 years of clinical experience in counselling or social work, especially in elderlies and related sector.
· Passionate about providing early palliative care to elderlies.
· Excellent collaboration and teamwork related skills.
· This position is on contract basis.
