- Plan and conduct on-site activities for seniors that promotes social interaction, and health and wellness in a group setting that meets performance indicators and guidelines issued by MOH
- To oversee the day-to-day operation and provide supervision
- Initiate a case record file for every senior household in the identified block and capture the needs of the seniors
- Establish networks with agencies and local community-based services for additional avenue of help for the welfare and to match the need of the seniors
- Oversee and manage the Alert Alarm System (AAS) during the operating hours, rendering assistance when necessary
- Maintain an updated register of seniors, volunteers and any other important records needed for follow up
- Conduct home care assessments and complete initial assessments (using Community Screening Tool) of senior and family to determine home care needs
- Ensure that the centre facilities and premises are in clean and good condition and maintenance is regularly executed
- Perform such other duties and responsibilities that may be assigned from time to time
Qualifications
- Minimum GCE “O” level with customer service background
- Well versed in Microsoft Office
- Competency in eldercare service with at least 2 years working experience in an eldercare
- Self-motivated team player who enjoys working with different profiles of seniors
- Able to plan, develop and implement programmes to meet the needs of seniors
- Able to engage, befriend and motivate seniors through facilitation of activities or programmes
