- To assist new patients with their applications and Know Right Start Right project
Main Resposibilities
- First contact for HD applicants
- Process new applications for the HD programme
- Conduct household Means Test and determine the subsidy quantum and welfare assistances
- Admissions Intake assessment of applicant’s Risks and Needs profile and referrals for assistance
- Create and update patient fee structure, welfare applications &Admissions intake in the system
- New patient DC Orientation
- Home visits
- Respond to MP letters/emails
- General admin duties
- Contact point for all restructured hospitals for HD applications
- Admissions and Social Work case handovers
- Managing of new patients who are deemed to be more challenging
- Attending to Know Right Start Right referrals
- Contribute and ensure that the department meet its KPIs
Qualification
- Degree / Equivalent in any discipline
Experience
- 1 - 3 years experience in social service / healthcare industry

- Dynamic, self-motivated and patient-oriented
- IT Savvy
- Team player
