Deepen quality of integrated person-centred care (PCC) to reframe and normalize life of PWD and PMCI, unleashing their hidden talents through visionary therapeutic engagements, build connections and provide opportunities for continuous learning and contributions to the Lodge as well as in the community.
Responsibilities:
- Co-create and enhance community care service streams for evolving profile of PWD and PMCI living in the community and their caregivers
- Lead, co-design and facilitate rich wellness activities virtually and in person to empower and unleash potential of PWD, PMCI and their caregivers
- Enthuse and synergise with knowledge partners, co-creators and family members to build a respectful and dementia friendly community to create person-centred care everywhere
- Strategize, plan and co-ordinate outreach and on-boarding of clients
- Co-creating knowledge from everyday best to build ground-up models with new benchmarks set
Requirements:
- A good basic degree in either Communication, Marketing, Psychology, Gerontology, or linguistic related field with 2 to 3 years of working experience
- Technology savvy and experience in social media marketing
- Strong in writing and documentation
- Possess keen minds and enjoy pathfinding work
- Creative, resilient, dynamic, organized, resourceful with good intrapersonal, interpersonal skills
