Senior Speech and Language Therapist
Salary: $5000 - $6000
Working Days: Mon - Fri (8:30am-5:30pm)
Working Location: Toa Payoh
A Senior Speech and Language Therapist is an advanced practitioner who works in a multidisciplinary team to ensure a high standard of clinical practice and professional conduct in all areas of client care.
1. Critical Work Functions
a) Conduct assessment on client and therapy planning
b) Evaluate history and assessment findings using clinical reasoning to develop an appropriate patient-centric intervention plan
c) Ensure comprehensive documentation where all relevant information related to client care (such as case history, assessment results, therapy plans, goals and outcome measures) is recorded and available to all members of the multi-disciplinary team
d) Formulate client-centred and SMART (Specific, Measurable, Attainable, Relevant, Timely) goals in collaboration with clients and relevant stakeholders
e) Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes
f) Communicate the assessment results, therapy plans, goals and outcome measures to clients and other relevant stakeholders
g) Work collaboratively with a dynamic multi-disciplinary team to provide clinically effective service to achieve client health and vocational goals
h) To act as a Return-to-Work Coordinator to support and facilitate client’s transition back to work
2. Perform Therapy Interventions and Client Education
a) Perform appropriate interventions based on findings, goals, and treatment plans
b) Evaluate therapy progress and outcomes using selected therapy outcome measures at regular intervals
c) Modify therapy plans based on client performance, motivation, safety as well as therapy outcome measures collected
d) Communicate the intervention plans, outcome measures, progress and discharge criteria to clients and relevant stakeholders
e) Educate clients’ families and employers about how to accommodate and support clients in their rehabilitation
f) Provide clients and caregivers with education and training in self-management
g) Assist to develop and implement client and caregiver educational materials and training programmes
h) Design and provide evidence-based and client-centred interventions (individual and groups) in collaboration with the multi-disciplinary team to improve client ability in functional domains, to promote reintegration to home, work and society
i) Perform field work involving visits to healthcare institutions, home, and worksite visits to support clients’ transition into employment and the community
Requirements
• Bachelor's Degree or equivalent in Speech and Language Pathology
• At least 5 years of work experience in a hospital setting
• A valid AHPC Practising Certificate
• Experience in working with population with neurological conditions would be an advantage
