Responsibilities:
- Provide safe, competent, and individualized nursing care, adhering to standards in documentation, treatment, patient monitoring, and infection control.
- Manage complex patient care with clinical skills, knowledge, and critical thinking.
- Monitor, reassess, and evaluate patient conditions, implement care plans, and support families coping with advanced diseases.
- Coordinate patient care with physicians, provide direct care, evaluate patient conditions, and liaise between families and doctors.
- Communicate with patients, families, or caregivers, assess home needs, and arrange equipment and aids.
- Allocate necessary equipment to meet care requirements.
- Ensure continuity of care and follow treatment plans.
- Administer medication, perform wound dressing, and document patient conditions accurately.
- Participate in multidisciplinary team meetings and collaborate with other teams for seamless care.
- Maintain patient safety and adhere to drug and medical supply storage regulations.
- Uphold confidentiality, use equipment properly, and manage resources economically.
- Handle public inquiries professionally.
- Follow AH policies and Singapore Nursing Board standards, and stay updated with industry guidelines.
- Maintain high housekeeping and workplace organization standards.
Requirements:
- Diploma in Nursing from a local Polytechnic, registered with the Singapore Nursing Board, with preferred post-registration experience in a hospital or hospice.
- Knowledge of IT systems.
- Passion for excellent customer service and detail-oriented.
- Strong interpersonal and communication skills.
- Good time management and teamwork skills.
- Ability to perform under pressure.
