Job purpose
- The Jobholder will provide direct social work case management, counselling to clients and their families, conducting social and financial assessment.
- Develop casework intervention plans, implementation and evaluation of programmes in order to help clients adapt to their limitations and assisting families with important decisions about their loved ones.
- Lastly uphold professionalism in social work and SSCH’s care philosophy of personcentered care.
Duties and responsibilities
- Review applications from AIC. Interview client’s and caregivers during enrolment into our programme/Home. Conducting Biopsychosocial assessment to assessing the strength and resilience of the client, families, and social support systems.
- Build rapport with residents/clients and their families. Maintain constant communication with clients while addressing their concerns and goals and helping them keep positive attitudes. And educating the family members on the physical and psychosocial needs of the clients and ways they can access internal and external resources.
- Provide casework management for our clients and coordination with external agencies such as hospitals and polyclinic for clients who require financial assistance for their medical appointments.
- Develop and implement individual care plans (ICP) for clients and provide therapeutic support as needed. – working with other multidisciplinary team (MDT) members.
- Execute the ICP and monitor progress. Develop and implement programmes and activities through a planned/structured manner that is aligned to SASCO values to meet the clients care needs.
- Coordinate with PT and OT departments to assist in helping clients feel comfortable and confident.
- Put up 6th monthly ICP review reports and monthly review records for clients.
- Evaluate existing, new and prospective clients based on their needs, desires and limitations.
- Update clients particulars and social report/review. Conduct casework evaluation, maintain case notes/documents.
- Person centred counselling for clients and/or families - counselling clients who suffer from poor mental health states (e.g. depression, anxiety), and coping and adjustment difficulties in their transition.
- Risk assessment - assessing risk of self-harm (e.g. suicide) and to others (e.g. family violence, elder abuse).
- Financial assessment and fund management and prepare periodic reports as required - identifying and referring cases for financial assistance (e.g. generate Means Test Subsidy Change Rate Report from MOH for invoices printing/Changed Subsidy report, IHDC Master
- Any other additional tasks assigned by the Management.
Qualifications & Skills Required
- A Degree in Social work, Registered Social Worker
- At least 4 years’ social work experience
- Prior experience in residential setting for the elderly
- Have good knowledge of government financial schemes
- Ability to work with different groups, elderly, stake-holders
- Excellent written and inter-personal communication skills
- Generate reports and statistics as required by MSF
