- Assist in carrying out administrative work in the delivery of advisory services to patients and their families.
- Processing of information and statistics on assistance through an electronic system
- Executive care advisory may be required to gather information from patients and their families and assist them with the financial assessments procedures.
- Conduct assessment / home visits when necessary.
- Liaise with relevant agencies / authority / patients family.
- Assure that all referrals are attended to by relevant personnel.
- Liaise with nurses on patient-related matters.
- Keep update records of patients ( electronic or written data)
Requirements:
- Minimum Diploma in Social Science or Social Work or above
- Minimum 1 year experience will be advantageous
- Able to start immediate or within short notice
