MAJOR DUTIES & RESPONSIBILITIES
(A) SPECIFIC (80%)
1. Reviews each patient’s medication chart or prescription, under supervision, for the appropriateness of the drugs prescribed and to contact the prescribing doctor when an intervention is needed. The review has to ensure that:
- i. All orders are clear and specific
- ii. Doses are prescribed within therapeutic limits
- iii. Any potential adverse reactions and interactions are identified
2. Fulfill all drug orders accurately and in a timely manner.
3. Dispense, under supervision, medications to patients, explaining the indications for drug therapy and instructions for administration.
4. Educates patient on the pharmacological and non-pharmacological aspects of their therapy. Check compliance and encourages adherence to medication therapy, where applicable.
5. Ensure that all sales and supply of medicinal products comply with legal requirements:
- i. Ensures records pertaining to all phases of drug receipt, supply , storage and distribution are maintained as required by the hospital and/or legal authorities.
- ii. Performs inspections of assigned drug storage areas in the hospital.
6. Ensures tha all related costs and revenue are accounted for and accurately recovered.
7. Provides drug information to patients and other healthcare professionals.
8. Responsible for the implementation and adherencfe to policies, practices and regulations established by the department, hospital and external regulatory agencies pertaining to pharmacy.
9. Represents the pharmacy on various committees and performs other job-related duties as assigned by the Chief, Pharmacy.
(B) GENERAL (20%)
1. Takes ownership and responsibility of own pre-registration training and participates actively in discussion.
JOB REQUIREMENTS
(A) Education / Training / Experience
Bachelor of Science degree in Pharmacy from an accredited Universty recognized by the Singapore Pharmacy Council
(B) Professional License
Registration with the Singapore Pharmacy Cpuncil to undergo pre-registration training
