As a Medical Social Worker, you provide support in offering services to our residents in the Nursing Home. Your goal is to identify their needs and develop interventions to meet those needs.
Job Description
The Medical Social Worker (MSW) will provide psycho-social assessment and interventions to support residents and their families as part of delivering person-centric care.
Residential Care
- Conduct resident’s pre-admission assessment with nursing staff.
- Attend multi-disciplinary meetings with nurses and other healthcare professionals to discuss, propose and coordinate care plans for residents.
- Design and develop relevant programs on psycho-socio care that promote quality care that look at well-being of residents.
- Liaise with organisations that provide services to meet residents’ and their families’ needs and refer them if necessary.
- Develop discharge care plan for clients if their health condition is fit for discharge.
- Identify gaps and streamline work processes within organization and service deliveries related to residents’ care.
- Advocate for changes with relevant stakeholders to enhance the care and well-being of residents.
Administration
- Administer renewals of residents' funding schemes when needed.
- Apply for Financial Assistance (e.g. Eldershield IDAPE, MFEC)
- Manage Medifund administration and applications in collaborations with Finance Department.
- Any other duties as assigned by the Reporting Supervisor
Supervision/Development of team members
- Manage and supervise team members
- Guide on the professional development of team members
Job Skills and Qualification
- Degree or Graduate Diploma in Social Work. Accredited Social Worker with SWAAB.
- Minimum 5 years of related working experience. Relevant experiences in Nursing Home/ILTC sector are an advantage.
- Passionate to work with seniors and their families.
- Excellent communication skills and ability to speak local languages.
- Ability to work well in a team.
- Good interpersonal and problem-solving skills.
- A fast and organized worker while able to multi-task.
- Proficient in MS Office.
