You will be an integral part of a multidisciplinary team providing post diagnostic support for families and children with developmental delays, disorders and / or special needs both in the clinic and community setting. You will play a critical role in supporting and engaging families through their journey from diagnosis of the child until they transit to other support services in the community. Duties will include:
Case work/ Psychosocial Care
- Being the touchpoint for families and making systems and services more accessible for them
- Carrying out regular screenings (both formal and informal) to identify caregivers and families who will benefit from the program initiatives at both clinic and home settings
- Providing support, case management and information for families and linking them with services available
Group Work
- Assist in conducting group work sessions for target groups
- Assist in delivering workshops and training programs for parents and caregivers
Program Development & Community Work
- Assist in conducting community needs assessment and community mapping
- Network with community stakeholders such as Social Service Organizations, MSF, etc. for effective care integration
- Supporting the overall planning and implementation of the program
- Developing forms, materials and resources for the program
- Assist in data entry, preparation of reports, collation of materials for meetings and taking minutes of meetings
- Required to provide training for community providers
Requirements:
- Degree in social work, with at least 1-2 years of relevant experience in either healthcare, community programs, early intervention or SPED school setting is necessary
- An independent, resourceful and compassionate team-player who is able to collaborate and multi-task in a dynamic environment
- Has strong interpersonal skills and proficient in MS Office such as Excel, Word and Access
- Knowledge of a 2nd language will be an advantage
