- Develop individualise care plans for residents.
- Execute the care plan and monitor progress.
- Coordinate care for residents with the relevant stakeholders.
Job Requirements :
- Diploma/Bachelor’s Degree in Social Work, Psychology or Counselling.
- Preferably 1 year of relevant experience in Social Service sector or Healthcare.
- Willing to perform shift work in accordance with the duty roster, Mon to Fri (staggered office hours)
- 8 hours: 8am to 5pm / 8.30am to 5.30pm / 9am to 6pm
- 9 hours: 8am to 6pm / 8.30am to 6.30pm / 9am to 7pm
