Provide trauma-informed and person-centric care for social and mental health needs through rigorous assessment, case management, and psychosocial and psychological interventions to children and youth with mild to moderate mental health needs. The purpose is to nurture a healing environment and facilitate the child/youth’s mental wellness journey in the community.
Job Description
Clinical
- Conduct triage to ensure that needs can be adequately managed in a community setting.
- Conduct assessment of medical, psychological, social, environmental, vocational and financial needs.
- Conduct risk assessment, safety planning and implement safety plans as appropriate.
- Formulate and develop intervention plans with children, youth and/or caregivers.
- Implement intervention plans and monitor progress.
- Review and evaluate the effectiveness of intervention plan with children, youths, caregivers and intervention team in a timely manner.
- Maintain up-to-date case documentation.
- Collaborate closely with families’ support networks, multi-disciplinary team and community partners.
Service Improvement
- Develop, implement and evaluate new strategies, procedures or systems.
- Participate in quality improvement projects.
- Opportunities to be involved in organisational projects.
Requirements
- Masters in Counselling.
- At least 3 years of relevant work experience in social service sector.
- Counselling experience and working experience with children, youth, or individuals struggling with mental health issues will be of added advantage.
- Willing to work on a flexible schedule and provide home-based interventions.
- Must be an insightful and reflective team player with passion in child/youth mental health, strong oral and written communication skills, effective interpersonal skills, high levels of initiative, adaptability and responsibility, and willingness to grow professionally with the organisation.
